Referral Form for Agencies & Churches

Dear Community Partner,

There is a verse in the Bible that reads, “Behold, I am doing a new thing; now it springs forth, do you not perceive it? I will make a way in the wilderness and rivers in the desert.” —Isaiah 43:19 (ESV)
As one immersed in the social service sector of our community, you may well know people that seem to go through life “wondering in the wilderness.” We know them too.

Starting in May the Urban Mission is launching a program called, “Fresh Start.”

This “new thing” has been in the works for a few years now, as we have been listening to you and those we serve and discerning the role our organization should play in helping to address the deeper issues associated with poverty.  The Fresh Start program is our attempt to help individuals and families find a fresh start in life in a more holistic, long term way. It is geared for those experiencing generational poverty with the goal of helping individuals and families break out of the cycles of hunger, homelessness, and unemployment.

It is not an “end all” program as we know it takes an entire community to help someone find a fresh start in life, but it is our way to take action and put what we preach into practice.
It’s also a work in progress and we are seeking your input into how best we can serve your organization and others in the community.

The first initiative under the Fresh Start program is to equip our community partners with a referral form for Urban Mission services. If you do not already have a referral form that you utilize, you are invited to use this one. It can also serve as a reference to you in regards to what services are offered by the Urban Mission.
